1) I set a value of an objective, let's say ItemPotionType, to 0 for anyone with a score of 1 or greater.
2) If someone is holding a certain item in their hand (i.e. {SelectedItemSl ot:{id:minecraf t:iron_sword}}) , I set the value of ItemPotionType to 1
3) If someone with the score ItemPotionType 1 and DamageDealt is greater than 0, grant a potion effect to anyone within a radius of 5 (or more, if you prefer) and a score of DamageTaken greater than 0.
4) Set the value of DamageTaken to 0 for anyone who's score is greater than 1.
5) Set the value of DamageDealt to 0 for anyone who's score is greater than 1.
The key to this is set the values AFTER you run the third command block, for otherwise the values will already be set to 0 before the command can run, and will never successfully run.
